Submission of this application is made on the understanding that any proprietary company information, if provided, shall not be divulged or disclosed without express permission in writing by company's authorized personnel. Furthermore, no supervision or certification, nor any authorization of the use of any kosher serice marks, is expressed or implied merely by submission of this application. Such authorization is effective only upon mutual contractual agreement in writing and issuance of formal certification letter. It is further understood that consideration of this application will involve plant visitation and/or other costs and that such costs will be disclosed to applicant in advance, and that applicant agrees to pay such costs regardless of whether application receives final approval. Pursuant to application approval, any such costs paid by applicant will be credited towards setup fee charges of the supervision contract.
